Staff and elder femaleThese are the home health services available at our agency. All these programs target the over-all health management of the individual.

ACTELL Elderly Care, Inc. believes it is possible to maintain ones independence at home. We offer services through Medicaid Waiver, several agencies, insurance companies, and long term care diversion programs.

Home Health Services:

Home Health Aide / CNA

elder femaleA Home Health Aide, under the direction of a RN or therapist assists you with activities of daily living, and may:

  • Help you with personal care (bathing, dressing, shampooing, brushing your teeth, toileting).
  • Make sure you are comfortable in bed or that your family can safely move you from bed to chair.
  • Perform various patient related chores.

Home Health

Home Health services are medically necessary services, which can be effectively and efficiently provided in the place of residence of a homebound member. Services include Skilled Nursing, Rehabilitation, home-health visits and Medical Social Worker.

Personal Care

Assistance with eating, bathing, dressing, personal hygiene, and other activities of daily living. This service includes assistance with preparation of meals, but does not include the cost of the meals themselves. This service may include housekeeping chores such as bed making, dusting and vacuuming, which are incidentals to the care furnished or which are essential to the health and welfare of the member, rather than the member’s family.

Respite (In-Home)

Respite Care is defined as relief or rest for a primary caregiver from the constant/continued supervision, companionship, therapeutic and or personal care, of a functionally impaired older person for a specified period of time. Respite care may include personal care, homemaker or companionship activities.

Attendant Care Services

Attendant care services are a combination of several long-term care services provided during a specific period of time. These services include Companion, Homemaker and Personal Care. These services may include supervision and socialization to the client, laundry, shopping, cleaning, bed making, dusting, vacuuming, bathing, dressing, personal hygiene or other activities of Daily Living. The enrollee has the ability to choose which services are to be performed during the time that the worker is in the enrollee’s home.

Light homemaking activities (meal preparation and light routine household care

Chore Services

Staffs and elder femaleServices needed to maintain the home as a clean, sanitary and safe living environment. This service includes heavy household chores such as washing floors, windows and walls, tracking down loose rugs and tiles, moving heavy items of furniture in order to provide safe entry and exit.

Adult Companion

Non-medical care, supervision and socialization provided to a functionally impaired adult. Companions assist or supervise the member with tasks such as meal preparation or laundry and shopping, but do not perform these activities as a discrete service. An adult companion does not entail hands-on nursing care. This service includes light housekeeping tasks incidental to the care and supervision of the enrollee. This service is contracted by Home Health Care Agencies.

Escort

Personal escort for members to and from service providers. An escort may provide language interpretation for people who have hearing or speech impairments or who speak a language different from that of the provider. Escort providers assist members in gaining access to services by public transportation. Not in Employee cars.

Bath Visit

Bath visit consist of a one hrs visit to shower or sponge bath the client. Bathing perform by a CNA or a HHA, for 1-2 hours of service for a set fee.
